Intervención educativa sobre conocimientos y prácticas enfocadas al uso racional de componentes sanguíneos en médicos del Hospital Baca Ortiz de Quito, 2017

Abstract

Context: There is a significant deficit of blood components in the country; the inadequate prescription contributes to hospital morbidity, which motivates the eradication of inap-propriate transfusions through education programs that have shown a positive impact on the optimization of resources and patient safety. One of the functions of the clinical pathologist is to direct the Transfusion Medicine Service, so it must promote the rational use of blood components. Objective: Educational intervention on the rational use of blood components in the Baca Ortiz Pediatric Hospital of Quito, in order to reduce the percentage of inappropriate transfusion prescription. Material and methods: Non-experimental pre and post-assessment design on a proposal for educational intervention for the rational use of blood components, based on the “Clinical Practice Guide, Blood Transfusion and its components” of the Ministry of Public Health, applied to 151 doctors; knowledge, practices and attitudes were assessed before and after the educational intervention by means of a structured structured written test. The analysis of the 467 blood transfusions performed at the Pre-intervention and post-intervention Hospital allowed the calculation of the percentage of inappropriate transfusion prescription. Statistical analysis was performed with the SPSS version 18 program. Results: In the trained doctors there was a gain of 38.5% in theoretical knowledge and 59.6% in attitudes and practices; overall the improvement was 62.9% (p <0.05, Chi2 McNemmar). In addition, the percentage of inadequate transfusion prescriptions decreased from 36.4% to 8.35% after the educational intervention (p <0.05, T for difference in pre and post-intervention proportions). Conclusions: The intervention contributed to the proper use of blood components, to the optimization of transfusion therapy and to patient safety at the Baca Ortiz Hospital.
