Pronóstico del cáncer papilar y folicular de tiroides
Resumen
El cáncer de tiroides ocupa un lugar prominente en cuanto a incidencia en él Ecuador, siendo los tipos papilar y folicular los frecuentes. En estos últimos, la amplitud de la cirugía y el tratamiento complementario con 1131 han sido motivo de permanente controversia a nivel universal debido básicamente a la falta de ensayos terapéuticos randomizados.
Tratando de adaptar el tratamiento a la extensión deía enfermedad se han propuesto varios índices pronósticos con sistemas de puntajes en base a los cuales proponer la estrategia de tratamiento. Utilizando el sistema AGES de la Mayo Clinic hemos encontrado que en una serie de 42 cánceres papilares y foliculares, los 30 casos considerados de bajo riesgo tuvieron una sobrevida actuarial de 100 ofo a 5 años en tanto que en los 12 casos de altó riesgo dicha sobrevida fue de 25 o)ó. En consecuencia, en este último grupo se justifica una terapéutica mas amplia y agresiva.
