Integral bucal rehabilitation en autism spectrum disorder patient. Case report

Abstract

Autism spectrum disorder (ASD) is a term used to describe a set of early-onset social disorders, affecting physical, social and language skills. Pediatric dental management in autistic patients is aimed at behavioral management techniques say-show-do, voice modulation, stabilizing protection and the use of pictograms. Clinical case report: Male patient, 5 years and 2 months old, with a diagnosis of ASD grade II, goes to the dental office due to the presence of increased volume on the lower right side of the face and an abscess at the level of the dental organ 84. It is observed, on oral examination, multiple carious lesions, giving early childhood caries as a diagnosis. A pulpectomy is started and a double is prescribed. antibiotic regimen of amoxicillin with clavulanic acid and metronidazole. In subsequent appointments, pulpectomy obturation (OD 84) is performed with calcium hydroxide paste with iodoform; control of dental plaque, indicating brushing technique and topical application of 5% sodium fluoride varnish and placement of a chrome-steel crown of the dental organ 84; complete oral rehabilitation, pulpectomy of dental organ 64 and chrome steel crown; extractions of dental organs 54 and 74; band-ansa space maintainers. A control appointment was made after 3 months, carrying out preventive treatments, placing high viscosity glass ionomers in dental organs 55, 65, 75 and 85. It was concluded that there were no trans- and post-operative complications, meeting the objective of children's oral rehabilitation. It is important that the pediatric dentist knows the care protocols for patients with autism spectrum disorder in order to avoid situations of rejection of dental treatment in patients who have this disorder, reducing the need to perform dental procedures under sedation or general anesthesia.
